This resource is intended to assist Maternal and Child Health (MCH) professionals in implementing the Pathways Community HUB (HUB) model, an evidence-based practice designed to improve health outcomes and reduce costs by connecting at-risk individuals to medical, social, and behavioral health services through community health workers (CHWs).
